9. Procedures for responding to requests for disclosure of retained personal data or records of provision to third parties

If you or your representative request notification of the purpose of use, disclosure, correction, addition, deletion, suspension of use, erasure, suspension of provision to third parties, or disclosure of records of provision to third parties regarding personal data held by our company, we will confirm that the person making the request is the individual themselves or their representative, and then respond within a reasonable period of time in accordance with the provisions of the Personal Information Protection Law.
Please follow the procedures below.

(1) Acceptance of disclosure requests, etc.

We accept inquiries regarding personal information by mail, telephone, or email at “10. Personal Information Inquiry Desk.”

(2) Documents to be submitted when requesting disclosure, etc.

After confirming your request, we will send you the necessary documents via our personal information inquiry desk. We will inform you of the delivery method separately via our personal information inquiry desk. Please fill out the necessary information on the documents we send you and submit them along with the confirmation documents (see (3) below) to “10. Personal Information Inquiry Desk.”

(3) How to verify the person making the request

 A. If the person making the request is the individual themselves
  One of the following forms of identification
  ・Driver's license
  ・Passport
  ・Alien registration card
  ・Basic Resident Registration Card (with photo)

 B. When the request is made by an agent  
   All of the following documents  
   • Identification documents for the individual listed in A above  
   • Identification documents for the agent listed in A above  
   • Power of attorney from the individual, or documents confirming that the agent is a legal representative, etc.

(4) Fees

No fees will be charged.

(5) Precautions regarding requests for disclosure, etc.

In cases where there is no obligation to respond under the Personal Information Protection Law, or if there are deficiencies in the request procedure, we may not be able to respond to your request.

The documents you send us when making a request will be used only to the extent necessary to respond to your request. We will not return the documents you send us.

We will respond by mail or e-mail to the address listed on the request form (within Japan) of the person making the request (or the representative, if applicable).

Even if we delete or erase personal information based on the above request, we will retain the request form, confirmation documents of the person who made the request, and a copy of the response form submitted at the time of the request.

Depending on the content of the request, it may take some time to respond.
